The Certificate of Borrower's Condition is a document that is required if you have had federal loans discharged due to a medical disability in the past but are applying for new loans.

View information on your personal loans, you will need to log onto Student Aid using your FSA ID.

Required Forms

  • To be certified for Federal Direct Loans at Capella University, you must submit an updated Learner Acknowledgment of Condition form that indicates your current condition allows you to engage in gainful activity so you can pay back any new student loans.
  • If your physician has not previously completed the Physician Certification of Borrower's Condition form, you will need to submit that form as well.
